Remote Patient Monitoring Virtual Care Program

North York General Hospital has launched a new virtual care program aimed at protecting patients and preventing hospital readmissions through remote monitoring. The new North York Toronto Health Partners (NYTHP) CareConnect program utilizes an app called Vivify, which helps monitor two patient populations: Depression and Anxiety for Mental Health, Falls Prevention for Frail and Elderly and COVID-19 positive patients. 

The purpose of this new virtual care application is to decrease the number of recurring visits to the Emergency Department through enhanced symptom monitoring and rapid care for both those with mental health concerns, frail and elderly patients who are more prone to falls and patients diagnosed with COVID-19.

This initiative received provincial funding to enhance the pandemic response through appropriate virtual care options, which are key in ensuring continuity of health services while preventing the spread of COVID-19.
